DFI Title XX Camper Service Applications for June and July/August
Please complete one of these applications for June 2008 and the other for July/August 2008. Send us one copy for each application, and keep a copy for your records. If you have additional programs with different per diems either call us to request additional applications or make a copy of the form from the ones enclosed
Cost Calculation Sheet for Resident Camper Nights and/or Cost for Day Camp
Instructions about calculating the per diem is enclosed.
Community Partnership Agreement
Each camp in this program needs to complete aCommunity Partnership Agreement with a local office of the IDHS. A copy of the Community Partnership Agreement is enclosed.
The purpose of this requirement is to assure that if a camp identifies a need in any camper eligible for DFI Title XX subsidy that the camp cannot meet the camp would make refer the camper to the IDHS office that serves that camper so that the unmet need can be addressed.
In most cases this will mean the camp must contact one or two IDHS local offices and request that a person authorized to sign from that office sign the enclosed Community Partnership Agreement.
Also enclosed is a list of local IDHS offices.
Requirement to name ACA Illinois as a named insured on your liability policy
Based upon the advice of our attorneys and our insurance consultant the American Camp Association, Illinois requires all camps that participate in the DFI Title XX Camping Services Program to add ACA Illinois as an additional insured under the camp’s general liability insurance policy as a funding source with respect to the camp’s operations.
Please contact your insurance agent, broker or company and ask them to name the American Camp Association, Illinois as an additional insured under your general liability policy and to send a copy of the certificate directly to us. Most underwriters who insure not for profit organizations are familiar with this type of request. Underwriters of for profit camps will be familiar with the form CG 20 26 used for this purpose. The suggested language could be “American Camp Association Illinois as funding sources with respect to the insured’s operations.”
